Patient and carer participation and empowerment

Clinical practice guidelines include as a fundamental component in improving the health management of patients with PMM (note: I’m not sure what «PMM» stands for; please provide the English equivalent if available) the empowerment of the patient and caregiver, aiming for them to play a more active role in their care. This empowerment is achieved through health education, participation in clinical decisions, and involvement in the entire care process by the patient. 

INCA provides a platform where patients have all their data at their fingertips and can manage them through the application to feel empowered and knowledgeable about their illness. Through gamification, we motivate patients to participate in their care by providing health tips or monitoring their symptoms through health challenges. These health content can be stratified by disease state levels so that the healthcare professional can tailor monitoring and health information to the patient’s needs. 

On the other hand, INCA offers a communication module for patients and caregivers with their healthcare team to ensure communication at all times.
